Curana Health is seeking a Nurse Practitioner or Physician Assistant to deliver high-quality primary care to seniors in skilled nursing, assisted living, and independent living communities. You will assess, diagnose, and manage a range of conditions with an emphasis on preventive care, autonomy, and patient relationships.

You’ll work with experienced physicians and care teams, travel to facilities, and leverage advanced technology to improve outcomes and keep residents thriving.

Qualifications

• Valid NP or PA license in state of operation and current board certification.
• Master's degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and ability to function within a team.

Responsabilités

• Perform comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent.
• Collaborate Across the Care Continuum with physicians, staff, facility leadership, specialists, and family members to coordinate comprehensive care plans.
• Maintain accurate and timely documentation within the EMR and ensure regulatory compliance.
