Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and current board certification in your field
• Master's Degree in Nursing or Physician Assistant Studies
• Strong leadership, interpersonal, and problem-solving skills preferred
• Excellent written and oral communication skills
• Punctuality, organizational skills, and ability to function within a team

Responsibilities

• Deliver high-quality primary care in senior living communities and facilities
• Perform comprehensive health assessments and develop individualized plans
• Diagnose, treat, and manage acute and chronic conditions with emphasis on preventative care
• Build meaningful patient relationships through regular rounds and collaboration with care teams
• Maintain accurate EMR documentation and ensure regulatory compliance
• Support initiatives to improve patient outcomes and reduce hospital readmissions
