Qualifications

• Must hold NP or PA license and board certification.
• Masters in Nursing or PA Studies.
• Strong leadership, interpersonal and problem-solving skills.
• Excellent written and oral communication.
• Willingness to travel to multiple facilities.

Responsibilities

• Provide direct patient care and primary care in SNFs and senior living communities.
• Perform comprehensive assessments and care management.
• Conduct routine rounds and adjust treatment plans.
• Coordinate care with facility staff and other providers.
• Educate residents and families on wellness and chronic disease.
• Maintain EMR documentation and regulatory compliance.
