Qualifications

• Valid nurse practitioner or physician assistant license in state of operations with current board certification.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Perform comprehensive assessments, develop treatment plans, and provide ongoing care management for residents.
• Conduct regular rounds to assess residents’ conditions, adjust treatment plans, and address emergencies.
• Collaborate with facility staff, family and other healthcare providers to ensure coordinated care.
• Educate residents, families, and facility staff on disease prevention, wellness, and chronic illnesses.
• Maintain accurate EMR documentation, ensuring regulatory compliance.
• Participate in quality improvement initiatives to reduce readmissions and enhance care.
