Qualifications

• Valid nurse practitioner or physician assistant license in state of operations.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Perform direct patient care and assessments in SNF and senior living facilities.
• Develop treatment plans and provide ongoing care management for residents.
• Conduct routine rounds, adjust plans, and address health concerns.
• Coordinate care with facility staff, families, and other providers.
• Educate residents and staff on disease prevention and wellness.
• Document accurately in EMR and ensure regulatory compliance.
• Participate in quality improvement initiatives to enhance care.
