Job summary

Curana Health is seeking a NP/PA to provide high-quality primary care across senior living communities and skilled nursing facilities. You’ll diagnose, treat, and manage conditions with a focus on prevention, while building meaningful relationships with residents, families, and care teams.

You’ll collaborate with physicians and leaders to coordinate care, document in EMR, and uphold standards in a mission-driven, growth-oriented organization that values autonomy and technology-enabled care.

Qualifications

• Active Nurse Practitioner or Physician Assistant license in the applicable state.
• Master's degree in Nursing or Physician Assistant Studies.
• Strong leadership and interpersonal skills.
• Excellent written and verbal communication.
• Able to work in a team and travel between facilities.

Responsibilities

• Deliver primary care in skilled nursing, assisted living, and independent living settings.
• Diagnose, treat, and manage acute and chronic conditions with preventative focus.
• Build relationships with residents, families, and care teams.
• Coordinate care with physicians and facility leaders.
• Maintain EMR documentation and comply with standards.
