Job summary

Los Angeles LGBT Center seeks a Senior Care Specialist II to support LGBTQ+ seniors across Los Angeles County, developing care plans and linking clients to housing and services.

The role emphasizes crisis intervention, documentation, collaboration with housing and community partners, and delivering group activities.

Applicants should have 2+ years in senior care management, a relevant degree, HIPAA knowledge, and bilingual English/Spanish preferred.

Qualifications

• 2+ years of senior care case management experience.
• Experience with LGBTQ community and urban/suburban settings.
• Bachelor-level degree in social work, gerontology or psychology.
• HIPAA knowledge and certification within 90 days.
• Fluent in English; bilingual English/Spanish preferred.
• Facilitation, collaboration and strong interpersonal skills.

Responsibilities

• Maintain an active caseload and complete documentation per procedures.
• Provide crisis intervention and de-escalation during client crises.
• Connect participants to emergency housing, mental health and other resources.
• Develop and monitor comprehensive care plans for housing stability.
• Collaborate with housing/navigation and community partners to support outcomes.
• Lead weekly group activities and Emergency Preparedness education.
• Attend trainings and contribute to staff development and supervision.
• Address client grievances and ensure client-centered resolutions.
