Job summary

Community First Solutions in Butler County, Ohio, is seeking a Care Manager to support seniors in the Elderly Services Program. You will assess needs, coordinate services, and develop personalized plans in collaboration with clients and caregivers, aiming to enhance quality of life and independence.

This hybrid, full-time role offers daytime hours, some fieldwork, and one in-office day per week. A Bachelor’s degree in social work or related field, plus experience in geriatrics, is required;

Qualifications

• Bachelor’s degree in Social Work, Gerontology, or related field; relevant experience may be considered in lieu of degree.
• 1 year of experience in geriatrics, case management, long-term care, home health or medical social work preferred.
• Valid driver’s license, insurance, and reliable transportation for home visits.
• Acceptable criminal records check required.
• Strong computer skills, including basic applications and care management software.

Responsibilities

• Assess client referrals for eligibility and enroll eligible individuals.
• Develop individualized service plans with client/caregiver, including ESP services.
• Provide information, referrals, and resources as needed.
• Inform clients of rights and responsibilities.
• Monitor service plan costs to ensure adherence to cost caps.
