Job summary

CJE SeniorLife is seeking a Care Manager to join the Care Management Services team. The role focuses on coordinating care for older adults, families, and caregivers, linking health and social services, and providing advocacy and direct support. Expect an autonomous workload with supervision as needed.

Responsibilities include assessments, referrals, and developing care plans to maximize independence and quality of life for clients in the community within a mission-driven setting.

Qualifications

• Graduate degree in gerontology, health, social work or related field.
• Minimum 2 years in a human service field; master's level in social work or related field acceptable.

Responsibilities

• Screen, assess, and document client needs and goals.
• Coordinate referrals to health and social services.
• Maintain caseload of ~35 clients with in-home assessments and care planning.
• Advocate for older adults during transitions between care settings.
• Develop care plans and provide in-home assistance when needed.
• Ensure timely clinical notes and adherence to agency policies.
