Responsibilities

• Build trusting relationships with members and families to support care coordination.
• Implement care plan activities and monitor progress toward goals.
• Maintain HIPAA confidentiality in all processes.
• Facilitate clear communication between internal teams and external clinicians.
• Identify and resolve barriers to care including transportation and financial barriers.
• Serve as patient navigator and primary point of contact for members and families.
