Job summary

Buckeye PACE is seeking a Master Social Worker to join One Senior Care’s team, focusing on relationships, listening, and guiding seniors and families through care plans. You will connect participants with resources and support them with empathy and a collaborative approach.

In this full-time role, you will participate in assessments, develop care plans, provide individual and family counseling, lead group activities, and advocate across agencies to strengthen well-being and independence at home.

Qualifications

  • MSW from an accredited school of social work with 1 year experience with frail elderly.
  • One year of experience with frail elderly or willingness to participate in training.

Responsibilities

  • Build trusting relationships with participants and families.
  • Participate in initial assessments and care planning with the interdisciplinary team.
  • Provide individual and family counseling in a supportive, person-centered manner.
  • Lead group discussions and activities to support well-being.
  • Listen deeply to concerns and guide toward resolutions.
  • Connect participants with community resources and advocate with outside agencies.
  • Liaise between participants, caregivers, team, and community partners.
  • Maintain case management records and timely documentation.
  • Support team cohesion across departments.
  • Participate in program development and training.
  • Communicate participant changes to the team clearly and collaboratively.
  • Other duties as assigned

Job Summary

As a Master Social Worker youll be at the heart of our participant experience forming relationships quickly listening deeply and helping seniors and their families navigate challenges with confidence and care Youll use empathy communication and a warm encouraging approach to guide participants through their care plans connect them with needed resources and help them feel supported every step of the way This role is ideal for someone who enjoys conversation collaboration and making a direct impact through people centered work

DutiesResponsibilities

  • Participate in initial assessments care planning and ongoing re evaluations as part of the interdisciplinary team
  • Provide individual and family counseling with an informal supportive and person centered approach
  • Lead group discussions activities and programs designed to strengthen emotional well being and engagement
  • Listen deeply to participants concerns and use your understanding to guide them toward helpful resolutions
  • Connect participants with community resources and act as their advocate with outside agenciesli>
  • Serve as a liaison between participants caregivers team members and community partners
  • Maintain written case management records and complete timely documentation
  • Support team cohesion by building strong collaborative relationships across all departments
  • Participate in program development quality initiatives and ongoing training
  • Communicate participant changes to the team in a clear upbeat and collaborative way

Education and Experience
  • Masters Degree from an accredited School of Social Work MSW with one year of experience working with the frailelderly population
  • One year of experience working with the frail and elderly population or willing to participate in extensive training as needed
Physical Requirements
  • Works inside the facility throughout the Participant and office areas
  • Sits stands bends lifts and move intermittently during working hours
  • Is subject to frequent interruptions
  • Must be alert for Participants safety
  • Is subject to lifting carrying and supporting Participants
