Job summary

One Senior Care in Ohio seeks a Master Social Worker to be at the heart of the participant experience, building relationships quickly, listening deeply, and guiding seniors and families through challenges with confidence and care.

You will use empathy and a warm, collaborative approach to support care plans, connect participants with needed resources, and help the interdisciplinary team deliver person-centered services with compassion.

Qualifications

• Master’s Degree in Social Work (MSW) required.
• One year of experience with frail/elderly population.
• Valid state driver’s license and reliable transportation.

Responsibilities

• Build trusting relationships with participants and families through open, warm, and empathetic communication.
• Participate in initial assessments, care planning, and ongoing re-evaluations as part of the interdisciplinary team.
• Provide individual and family counseling with an informal, supportive, and person-centered approach.
• Lead group discussions, activities, and programs designed to strengthen emotional well-being and engagement.
