Job summary

Alder is seeking an Aging Life Care Professional to conduct comprehensive client assessments and develop goal-oriented care plans for seniors in Lower Fairfield County. The role involves field visits, collaboration with care teams, and proactive care coordination.

Flexibility, travel, and the ability to work from the Mamaroneck, NY office or remotely are required. Strong communication and senior care expertise are essential for success.

Qualifications

• Advanced degree preferred and licensure required in social work, nursing, geriatrics, mental health, or related field.
• 3-5 years’ experience working with older adults and families.
• Excellent clinical skills; knowledge of family dynamics and family systems.
• Outstanding interpersonal and communication skills.

Responsibilities

• Conducts comprehensive assessments of clients’ needs across functional, cognitive, medical, social and emotional domains.
• Assists clients and their care team with decisions on aging, disability, services, and long-term care planning.
• Creates goal-oriented care plans to address concerns.
• Responds quickly to emergencies and high-risk situations.
• Presents recommendations to the client and care team, involving all relevant parties in decision-making.
