Job summary

GroundGame.Health seeks a Lead Care Manager to identify and engage members in CalAIM ECM, CHI, CHW, and other care coordination programs. You will work as an extension of provider offices and GroundGame.Health, coordinating assessments and connections to healthcare and community resources.

Responsibilities include supervising CHWs, ensuring timely documentation in EHRs and Implify, and meeting program goals.

Qualifications

• CHW Certification or a minimum of two years in care coordination or related healthcare roles.
• Experience using Electronic Medical Records (EMRs).
• Willingness to travel between locations and work onsite in provider clinics, homes, and facilities.

Responsibilities

• Identify, enroll, and support members in ECM/CHI/CHW programs embedded in provider practices.
• Coordinate care plans, referrals, and follow-up using EMRs and GroundGame.Health tools.
• Lead and mentor CHWs, monitor performance, and ensure compliant documentation and billing.
