Job summary

GroundGame.Health is seeking a Lead Care Manager to identify, enroll, and support members in CalAIM ECM, CHI, CHW, and related care coordination programs. This role is embedded within provider practices as an extension of both the office and GroundGame.Health teams.

You will coordinate assessments, develop care plans with interdisciplinary teams, and ensure timely referrals and connections to healthcare, community, and government resources. The onsite location is Escondido, CA.

Qualifications

• CHW certification or at least two years in care coordination or related healthcare roles.
• Experience using Electronic Medical Records (EMRs).
• Valid driver's license with acceptable driving record and ability to travel between sites.
• Onsite work in clinics and patients' homes.
• Bilingual in English and Spanish.

Responsibilities

• Lead and supervise a team of CHWs assigned to the provider practice(s).
• Identify, engage, enroll, and support ECM/CHI/CHW participants.
• Coordinate care plans, referrals, and scheduling with interdisciplinary teams.
• Ensure accurate, timely documentation in EHR and Implify platforms.
