Job summary

CHRISTUS Health is seeking a Care Manager III to lead case management and discharge planning in collaboration with physicians and a multidisciplinary team. The role focuses on optimizing patient flow, transitions, and length of stay while ensuring patient safety and regulatory compliance.

The CM III coordinates care across departments, interfaces with external service providers, and advocates for patients to secure appropriate post-acute options and resources.

Qualifications

• BSN or MSW required or demonstrated success in CHRISTUS Care Manager II position for at least 5 years on top of the required experience (in lieu of education).
• Demonstrated leadership skills - formal or informal.
• Demonstrated problem-solving skills.
• LMSW accepted for associates with 5+ years in CM II within CHRISTUS Health.

Responsibilities

• Coordinates integration of case management into discharge planning with other departments and external agencies.
• Facilitates patient care progression through continuum of care in a cost-effective manner.
• Acts as patient advocate by negotiating resources with payors, agencies, and vendors.
• Initiates discharge planning at admission and makes post-hospital referrals as indicated.
• Documents in the medical record per regulatory and department guidelines.
