The Dauphin County Area Agency on Aging is currently accepting applications for the full-time, day shift position of Aging Care Manager 1. The hourly rate for this position is $20.00 per hour.

This position is covered under a collective bargaining agreement. The Aging Care Manager 1 develops care plans for older adult consumers after evaluating social contacts, activities of daily living, cognitive status, health, and resources, and coordinates with social service agencies to ensure appropriate

Qualifications

• Bachelor's degree includes or is supplemented by 12 college-level credit hours in sociology, social welfare, psychology, gerontology, or related social sciences.
• Or 3.5 years of experience as an Aging Case Aide and an associate degree with 12 related credits.
• Or any equivalent combination of experience and training and an associate degree including 12 related credits.

Responsibilities

• Develop and maintain care plans for older adult consumers through evaluation of social contacts, activities of daily living, cognitive status, health, and resources.
• Develop and maintain professional relationships with social service agencies and community resources to meet client needs and ensure appropriate continuum of care.
• Supervised by Aging Care Management Supervisor 1.
