HomeWell Care Services – Carrollton is seeking a Care Manager & Community Relations Liaison to join our growing team. This role blends care management, client support, community relations, and business development, requiring comfort building professional relationships with healthcare and senior-care organizations.

We are not a traditional sales role; we seek someone who can build trust, understand care needs, and develop meaningful community partnerships that lead to new client opportunities.

Qualifications

• 2+ years of experience in senior care or related field.
• Strong communication and relationship-building skills.
• Ability to balance care responsibilities with business development.
• Must have valid driver's license and reliable transportation.

Responsibilities

• Meet with prospective clients and families to understand care needs and goals.
• Participate in client assessments and care planning.
• Follow up with clients and families and monitor satisfaction.
• Communicate with families, caregivers, and referral partners regarding client needs.
• Help families understand home-care options and resources.
• Maintain accurate client and care-management documentation.
• Coordinate with the HomeWell office team to ensure smooth transitions from inquiry to start of care.
• Build and maintain relationships with referral sources and community partners.
• Make regular in-person visits to referral sources.
• Educate community partners about HomeWell's services.
• Identify opportunities for presentations, events, partnerships.
• Represent HomeWell at events.
• Track community contacts, referrals, and opportunities.
