Job summary

Qualifications

• 5 years of clinical case management experience or an equivalent background.
• Current, unrestricted LVN license in the state for which you are applying.
• Associate degree preferred.

Responsibilities

• Manage and coordinate care for members with short-term chronic care needs based on a plan provided by providers.
• Coordinate and monitor member progress and services to ensure cost-effective, compliant care.
• Interface with PCPs, Nurse Practitioners, RN case managers and specialists for coordination of care.
• Counsel and engage patients and families on available care options; provide follow-up to implement care plans.
