Job summary

Blue Shield of CA is seeking a Senior LVN, Care Manager to support outreach and enrollment by assessing member needs and coordinating care. You will engage with members, connect them to health programs, and collaborate with care teams in a hybrid virtual work environment.

Ideal candidates have 5+ years in nursing or healthcare, CA LVN license, CCM certification or in progress, and strong communication skills for diverse populations.

Qualifications

• High School Diploma/GED and completion of a CA LVN program.
• Current and unrestricted CA LVN license.
• CCM Certification or in process of completion when eligible.
• At least 5 years of nursing, healthcare or related field.
• Experience working at a health plan (managed care, government payer, or academic).
• Minimum 3 years in inpatient, outpatient, and/or managed care.
• Excellent communication skills.
• Bilingual preferred.
• Excellent interpersonal skills with diverse populations.
• Ability to independently assess, evaluate, and interpret clinical information.

Responsibilities

• Reach out to members, assess needs, and coordinate care.
• Provide comprehensive care coordination in outpatient settings.
• Engage members with appropriate health programs and resources.
• Communicate effectively with diverse populations and teams.
