Qualifications

• Active LMFT license in the state where applying.
• Excellent oral and written communication in English.
• Proficiency with electronic health records (EHR).
• Strong organizational skills with attention to regulatory compliance.
• Ability to provide in-person services at facilities (up to 45 minutes).
• Ability to move between rooms and offices within a facility.

Responsibilities

• Conduct individual and group psychotherapy sessions in subacute or skilled nursing facilities.
• Assess, diagnose, and treat emotional and behavioral conditions including depression and anxiety.
• Develop and implement evidence-based treatment plans tailored to patients.
• Collaborate with facility staff, interdisciplinary teams, and families.
• Maintain accurate clinical documentation in an EHR system.
• Monitor patient progress and adjust treatment plans as needed.
