Job summary

The Alliance in Central California seeks an experienced Senior Care Integration Nurse (RN) for a temporary, full‑time role. You will lead ECM/CS/TOC care coordination while ensuring regulatory compliance and effective provider training.

Hybrid work is expected with some in-person engagement; licensure in CA is required and a strong background in outpatient care is essential. Temporary status includes agency staffing arrangements.

Qualifications

• Current unrestricted RN license issued by the State of California.
• Associate's degree in nursing with a minimum of seven years of outpatient medical, care management, community health service delivery, behavioral health practice management or related healthcare experience; or a Bachelor's degree may substitute for two years of experience.

Responsibilities

• Provide Enhanced Care Management (ECM), Community Supports (CS), and transitions of care (TOC) support and care coordination for Alliance members.
• Monitor and audit ECM, CS, and TOC provider performance for quality care.
• Coordinate with internal and external teams to support seamless transitions across the care management continuum.
• Act as liaison between ECM/CS program and external providers, community agencies, and Alliance departments.
• Assist in ECM/CS program implementation, oversight, reporting, and provider training to meet regulatory requirements.
• Apply the principles of community-based nursing and service delivery to at-risk populations.
