Job summary

Central California Alliance for Health is seeking a Clinical Care Coordinator to provide Enhanced Care Management, Community Supports, and transitions of care for Alliance members. You will work under the Care Management Director, coordinating with internal teams and external providers to ensure timely access to care.

The role requires an active California RN license, a nursing degree, and substantial experience in outpatient care management.

Qualifications

• Current unrestricted RN license issued by the State of California.
• Associate degree in nursing with a minimum of seven years in outpatient medical or care management
• Bachelor's degree may substitute for two years of required experience

Responsibilities

• Provide ECM/CS/TOC care coordination to Alliance members.
• Monitor ECM/CS/TOC provider performance for quality and non-duplication.
• Coordinate care across internal and external teams and agencies.
• Liaise between ECM/CS program and external providers and departments.
• Assist in program implementation, oversight, reporting, and training.
