Job summary

Senior Community Care of Colorado PACE in Montrose, CO is seeking an Enrollment Specialist to support admissions, coordinate intake, and provide information and referrals to community agencies. This role serves the VOANS Senior CommUnity Care program and helps promote the program to potential participants.

The position requires a Bachelor's degree or equivalent experience with healthcare exposure, a driver’s license, and at least one year of work with the elderly.

Qualifications

  • Bachelor's degree or equivalent with healthcare-related experience.
  • Experience in marketing, sales or admissions within health care.
  • Experience with frail or elderly populations preferred.

Responsibilities

  • Promotes the Senior CommUnity Care Program to potential participants and agencies.
  • Assesses potential participants in homes, nursing facilities or hospitals.
  • Assist with financial eligibility processes for participants.
  • Schedule enrollment meetings with potential enrollees.
  • Review Medicaid eligibility and support re-certification and applications.

Salary Range: $58k - $65k annually, based on previous work experience as related to this role

Schedule: Monday-Friday 8am-5pm, occasional weekends

What You Bring (Requirements)
  • Bachelor's Degree in humanities/health and human services or equivalent experience with CNA or LPN certification.
  • Two (2) years of experience in marketing, with more emphasis on sales, and/or admissions role for a health care provider, or in an information and referral resource role within the senior networks. Shall have either training or related experience in the job assigned.
  • Have a minimum of one year of experience in working with the frail or elderly population.
  • Must have a valid driver's license and have means of transportation within one hundred (100) miles or more daily.
What You’ll Be Responsible For

The Enrollment Specialist obtains initial information relating to admissions, coordinates intake process, and provides information and referrals to community agencies as appropriate. Promotes the Senior CommUnity Care Program to potential participants and community agencies.

  • Assesses potential participants in home, nursing homes, hospitals or other settings as part of the intake process.
  • Assists with financial eligibility process for participants.
  • Acts as the point person who schedules the enrollment meeting with the potential enrollees.
  • Reviews Medicaid eligibility, monitors time frame for re-certification and facilitates Medicaid applications for certification and re-certification.
  • Initiate and complete enrollment activity to ensure that budgeted census is achieved. Specific goals will be determined by the Outreach and Enrollment Manager.
  • Other duties as necessary to ensure that marketing department goals are achieved.
  • Performs related duties as required or requested.
