BeeHive Homes of Vernal is seeking a dedicated Care Coordinator to manage daily operations within our elder care home. The role focuses on coordinating care services, advocating for residents, and ensuring timely treatment and support across settings.

The ideal candidate has a CNA certification with at least two years of experience, strong communication and organizational skills, and a compassionate, patient-centered approach to care.

Qualifications

• CNA Certification with 2+ years experience.
• Experience in care coordination or managing others preferred.
• Strong understanding of healthcare systems, processes, and protocols or willingness to learn.
• Excellent communication and interpersonal skills to build rapport with patients and families.
• Ability to handle sensitive and confidential information with discretion and compassion.
• Exceptional organizational skills with multitasking and prioritization.

Responsibilities

• Coordinate care: liaison among patients, healthcare providers, and others to deliver coordinated care.
• Report needs to RN with Admin to develop personalized care plans addressing physical, emotional, and social well-being.
• Advocate for patients, ensuring rights are respected and voices heard during interactions with providers.
• Coordinate services with RN and admins to organize treatments and follow-ups, monitor delivery and resolve issues.
• Document and report patient interactions, care plans, assessments, and referrals; review outcomes with RN to evaluate effectiveness.
