Qualifications

• CNA certification with at least 2 years of experience.
• Experience in care coordination or case management preferred.
• Knowledge of healthcare systems, processes, and protocols.
• Excellent communication and interpersonal skills.
• Ability to handle sensitive information with discretion and compassion.
• Strong organizational and multitasking abilities.
• Patient-centered focus with empathy.

Responsibilities

• Coordinate care: manage and coordinate care services by liaising with patients, healthcare providers, and other involved parties.
• Report needs to RN: determine needs and develop personalized care plans with RN & Admin.
• Advocate for patients: ensure rights are respected and voices heard during interactions with providers and stakeholders.
• Service coordination: organize treatments and follow-ups with RN and administrators; monitor quality and resolve issues.
• Documentation and reporting: maintain records of patient interactions, care plans, assessments and referrals; evaluate outcomes with RN.
