Job summary

Heritage Senior Living is seeking a compassionate Wellness Coordinator to lead the resident care team in Monona, WI. You will coordinate daily care, monitor ISPs, and support CNA/Resident Assistants to maintain high-quality, individualized services.

Responsibilities include directing rounds, managing staffing, documenting vitals and wellness data, and collaborating with the DON and families. This role blends hands‑on care with leadership to ensure exceptional resident experience.

Qualifications

• High school diploma or GED required.
• CNA certification preferred but not required.
• Experience in senior living or caregiving role is strongly preferred.

Responsibilities

• Coordinate daily resident care and rounds.
• Lead and support CNA/Resident Assistants on shifts.
• Coordinate staffing based on resident needs and ISPs.
• Train, mentor, and coach care staff; support new orientation.
• Monitor vitals, weights, and wellness indicators; document accurately.
• Coordinate showers, baths, and laundry per ISPs and resident preferences.
• Coordinate hospice and external provider visits and information sharing.
• Communicate with residents, families, and care team; escalate issues promptly.
• Provide hands‑on support and maintain resident independence where possible.
