Job summary

WellStar Health System is seeking a Care Coordination Social Worker Sr. for our Day shift in Georgia. The role centers on psychosocial assessments, discharge planning, and coordinating care across the continuum with patients and families.

You will guide transitions to the next level of care and support end-of-life discussions where appropriate. Ideal candidates hold a Master’s in Social Work with LMSW or LCSW in GA and may have ACM/CCM.

Qualifications

• Master's Degree in Social Work from an accredited college or university is required.
• LMSW in GA can be waived if you have an LCSW.
• ACM or CCM certification is preferred.

Responsibilities

• Assess psychosocial risk factors and resources for discharge planning.
• Collaborate with patient, family, physician and care team to establish care progression and discharge plans.
• Coordinate discharge planning and post-acute referrals for timely transition.
• Provide financial needs assessment for follow-up care.
• Precept and mentor new social workers and students.
