Wellstar Health System in Georgia seeks a Care Coordination Social Worker Senior to assess psychosocial needs and coordinate discharge plans across the continuum. You will consult with the care team on complex cases, guiding patients and families toward appropriate post-acute placements and services.

The role includes mentoring social workers, addressing social determinants of health, and collaborating with physicians to ensure patient-centered discharge planning.

Qualifications

• Bachelor's or Master Social Work required/preferred.
• Strong communication and assessment skills.
• Ability to coordinate discharge planning and resources.

Responsabilités

• Discharge planning for complex patients in the acute care setting.
• Identify barriers to timely disposition and document them.
• Understand local, state, and federal resources for patient transition.
• Respond to referrals from RN Care Coordinators and physicians.
• Participate in Interdisciplinary Rounds to confirm discharge date.
• Initiate post-acute referrals for timely transition to next care level.
• Provide financial needs assessment for follow-up care.
• Advocate for patient and family autonomy in care decisions.
• Honor cultural or religious beliefs in service delivery.
• Mentor and precept new social work hires and interns.
