Job summary

Wellstar Health System is seeking a Sr. Care Coordination Social Worker to assess transitional care needs and coordinate discharge planning across the continuum.

You will serve as a resource on psychosocial and resource needs, guiding patients and families toward appropriate post-acute options and supports. The role emphasizes collaboration with physicians, RN Care Coordinators, and the care team to ensure safe, patient-centered discharge planning.

Qualifications

• Bachelor's degree in Social Work required; Master’s preferred.
• Minimum 2 years of healthcare social work experience in an acute care setting.
• Excellent written and verbal communication skills.

Responsibilities

• Implements discharge planning and provides resource information for complex patients.
• Identifies and documents barriers for timely disposition.
• Collaborates with the care team to support discharge plans.
• Mentors new social workers and students in case reviews.
• Completes mandatory education and participates in protocol development.
