Job summary

You will collaborate with patients, families and physicians to ensure timely, safe transitions and to address psychosocial and resource needs. You will provide expert guidance on end-of-life planning, social determinants of health, and community resources, while mentoring new social workers and contributing to quality

Qualifications

• Excellent written and verbal communication skills.
• Ability to assess psychosocial risks and plan discharge.
• Strong collaboration with care teams and families.
• Experience mentoring and precepting staff or students.

Responsibilities

• Assess transitional care needs and coordinate across the continuum.
• Develop timely, safe discharge plans with patient and family input.
• Serve as a resource on psychosocial and discharge issues for the care team.
• Mentor junior social workers and participate in trainings.
• Document assessments and plans in the electronic health record.
