Job summary

Wellstar Health System in Georgia is hiring a Care Coordination Social Worker to assess patient needs, coordinate care across the continuum and support discharge planning.

In this role you will collaborate with physicians, nurses and social work colleagues to connect patients and families with community resources, ensure timely transitions, and advocate for patient preferences while maintaining compassionate, patient-centered care.

Qualifications

• Bachelor or Master of Social Work required, GA preferred.
• BLS or ARC-BLS or BLS-Instructor certifications required upon hire.
• LCSW or LMSW GA preferred.

Responsibilities

• Provide psychosocial assessment and timely discharge planning.
• Assist in coordinating post-discharge resources and referrals.
• Collaborate with care team to support patient needs and goals.
• Participate in Interdisciplinary Rounds to optimize discharge date.
• Document in EHR and track progress toward outcomes.
• Advocate for patient and family preferences in care decisions.
• Support end-of-life planning when appropriate.
