ArchWell Health is seeking a licensed social worker to support elderly patients within our Sarasota clinics. You will collaborate with primary care teams to assess social needs, plan supportive packages, and navigate community resources and benefits to remove barriers to care.

Ideal candidates hold an MSW with 1+ year clinical social work experience, strong communication, empathy, and PC skills; Spanish language ability is a plus.

Qualifications

• Fully licensed BSW or MSW in the state of practice.
• MSW preferred; Bachelor's degree required.
• At least 1 year of clinical social work experience.
• Proficient PC skills.
• Spanish or other languages helpful in service area.

Responsabilités

• Develop relationships with primary care teams to conduct social services assessments.
• Plan, coordinate, manage and implement support packages to address barriers.
• Navigate managed care plans for community services and programs.
• Case management for social and behavioral care to enable self-management.
• Document patient encounters accurately in clinical management systems.
• Identify opportunities to improve care based on feedback and surveys.
• Support advance care transitions with members and families.
• Assist members with access to state-based prescription programs and other benefits.
