Job summary

Consumer Direct Care Network seeks a Service Coordinator Supervisor to manage intake and referrals, coordinating in-home care for Medicaid clients in line with regulations.

You will supervise caseloads, provide leadership and training to staff, and collaborate with providers to ensure timely, high-quality service delivery. Strong communication, problem solving and customer service are essential; bilingual abilities are preferred in various locations.

Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, Rehabilitation, Nursing, Sociology or related field preferred.
• 3 years experience working with individuals with disabilities or the elderly preferred.
• Combination of education and experience.
• Leadership experience preferred; ability to work flexible/extended hours as needed.

Responsibilities

• Supervise and maintain a caseload of Medicaid clients.
• Ensure compliance with Federal/State regulations and MCO program rules.
• Oversee intake and referral processes to support growth.
• Provide leadership and training to staff.
• Collaborate with stakeholders regarding service delivery.
• Communicate effectively and maintain client advocacy.
• Conduct necessary client assessments and home visits as needed.
• Handle escalated complaints.
