Responsibilities

• Identify, build, and maintain relationships with referral partners (senior living communities, realtors, estate attorneys, hospice providers, healthcare organizations).
• Follow a consistent outreach plan to establish trust and generate qualified leads.
• Maintain regular contact via in-person visits, calls, emails, networking events, and community activities.
• Represent Caring Transitions professionally at all times.
• Maintain referral and lead tracking; provide weekly/monthly activity reports.
