Job summary

Trinity Health seeks a Senior Care & Community Outreach liaison to grow awareness and referrals for our PACE program. You’ll build referral partnerships with hospitals, physicians, rehab centers, and community groups to connect seniors with life-changing care.

This on-site role runs 8-5, Monday to Friday, with a $7,500 sign-on bonus and day-one benefits. You will educate communities, generate qualified referrals, and collaborate with enrollment and clinical teams to support seamless transitions.

Qualifications

• 2+ years in healthcare sales, outreach, or business development.
• Experience in senior care, home care, hospice, or PACE.
• Strong networking and presentation skills.
• Self-motivated with ability to manage a territory independently.
• Valid driver’s license and reliable transportation.
• Comfortable in assisting seniors in their homes.
