Job summary

Trinity Health PACE in Philadelphia is seeking a Senior Care & Community Outreach liaison to expand awareness and referrals for our PACE program. This on-site role emphasizes building relationships with hospitals, physicians, and community organizations, educating partners about program benefits, and identifying eligible seniors for services.

Qualifications

• 2+ years of experience in healthcare sales, outreach, business development, or community engagement.
• Experience in senior care, home care, hospice, PACE.
• Strong networking and relationship-building skills.
• Excellent communication and presentation skills.
• Self-motivated with ability to manage a territory independently.
• Comfortable in assisting seniors with the process in their homes.
• Valid driver’s license and reliable transportation.
