Job summary

Trinity Health PACE is seeking a Senior Care & Community Outreach liaison to grow awareness and referrals for our PACE program. This on-site role runs 8-5, Monday to Friday, with a $7,500 sign-on bonus and Day One benefits.

Responsibilities

• Develop and maintain referral relationships with hospitals, physicians, rehab centers, senior centers, and community organizations

• Maintain territory plans and referral pipelines

• Valid driver’s license and reliable transportation
