Job summary

Valley of the Sun YMCA is seeking a Part-Time Client Coordinator to support older adults at home and in the community. You will conduct in-home intake, assess needs, coordinate services, and connect clients and families with resources.

You will build relationships with clients, families, volunteers and partners, recruit volunteers, plan social activities, and help implement programs that promote independence and quality of life.

Qualifications

• Bachelor's degree in social work, Gerontology, Human Services, or related field preferred.
• Two (2) years of relevant experience may be considered.
• Experience with older adults in home or community settings is a plus.

Responsibilities

• Conduct in-home client intake and comprehensive needs assessments.
• Monitor client concerns and changes in abilities; coordinate follow-up.
• Schedule follow-ups and connect clients to support services.
• Maintain accurate client records and documentation.
• Provide care coordination with families, volunteers, and providers.
• Recruit and support volunteers for transportation, shopping, and visits.
• Lead Client Committee and engage clients to identify needs and opportunities.
