Job summary

Institute on Aging Southern California, LLC is seeking a temporary Care Manager/RN to perform comprehensive nursing assessments for participants in Riverside and San Bernardino Counties. The role coordinates care with a multidisciplinary team and develops plans to support aging at home within the HCBA Medi-Cal Waiver program.

Requirements include CA RN license, BLS, and significant experience in acute care and home-based waiver services.

Qualifications

• Active RN license in California (unencumbered).
• BLS certification required.
• Willingness to travel throughout Riverside and San Bernardino Counties.
• Experience in care coordination and documentation.

Responsibilities

• Conduct comprehensive nursing health assessments.
• Develop goals with participants and care teams.
• Create Plan of Treatment to meet client needs in the community.
• Identify and train backup caregivers for waiver services.
• Provide information, counseling, and advocacy to participants.
• Schedule care coordination based on needs and reassessments.
• Monitor waiver service delivery and outcomes.
• Maintain monthly client contact and periodic reassessments.
• Follow up after ED or hospital admissions.
• Perform other duties as assigned.
