Qualifications

• Bachelor’s degree in Human Services or related field plus one year of Case Management experience.
• Valid Florida Driver’s license and insurability by the company’s insurer.
• Background checks/clearance for working with vulnerable populations.

Responsibilities

• Coordinate services for functionally impaired older persons using client-centered principles.
• Conduct home visits and telephone calls to assess safety and needs.
• Review care plans and perform semi-annual/annual reviews.
• Maintain updated client records and timely care coordination documents.
• Engage in community outreach and partner with caregivers to build informal support networks.
