Job summary

Actalent is seeking a Care Manager / Case Manager to join our team in Wisconsin. You will manage an assigned caseload, conduct comprehensive assessments, and develop member-centered care plans in collaboration with an RN Care Manager.

Responsibilities include ongoing care coordination, documentation, and ensuring compliance with Medicare/Medicaid programs. Travel within Rock County is required; a valid driver’s license and personal vehicle are needed.

Qualifications

• Master’s degree in Social Work required; the master’s degree must be in Social Work.
• Current Wisconsin Social Work licensure required.
• Minimum of 1 year of experience providing care or services for a frail or elderly population.
• Ability to complete comprehensive assessments and member-centered care plans.
• Experience in care coordination or case management within healthcare or social services.
• Knowledge of Medicare, Medicaid, managed care and related programs.
• Proficient in accurate documentation and adherence to standards.
• Ability to collaborate with an RN Care Manager and a multidisciplinary team.
• Valid driver’s license, access to a personal vehicle, and insurance for travel.

Responsibilities

• Manage an assigned caseload of program members with an RN Care Manager.
• Complete comprehensive assessments including physical and psychosocial evaluations.
• Develop and update member-centered care plans based on needs and goals.
• Conduct functional screens and ensure timely completion of assessments.
• Provide ongoing care coordination and responsive services.
• Monitor quality of services and adjust care plans as needed.
• Provide direct social work case management, advocacy, and resource linkage.
• Facilitate team-based care management with interdisciplinary meetings.
• Maintain documentation per standards, policies, and regulatory requirements.
• Document services and track progress using healthcare management systems.
• Ensure compliance with Medicare, Medicaid, and related programs.
• Engage members, families, and caregivers in care planning to promote autonomy.
• Assist members in navigating community resources to maintain independence.
• Travel locally within Rock County using a personal vehicle; maintain valid license and insurance.
