Job summary

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to manage a panel of enrolled seniors in long-term care settings. You will conduct comprehensive assessments, develop individualized care plans, and work with an interdisciplinary team to advance goals of care and reduce hospitalizations.

You'll collaborate with families, participate in care team meetings, and ensure documentation and compliance with state licensure.

Qualifications

• 6+ months experience as a Nurse Practitioner or Physician Assistant.
• MSN as NP or Master’s degree from an accredited program.
• Experience in nursing home environments preferred.
• Proficient with computer technology and documentation.
• Strong communication with physicians, members, and families.

Responsibilities

• Provides collaborative primary care to a panel of members.
• Conducts comprehensive assessments and develops individualized care plans.
• Participates in nursing facility care team meetings and coordinates with families.
• Maintains communication with members’ PCPs and interdisciplinary team members.
• Ensures high-quality, member-centric care and supports care team goals.
