Job summary

Bluegrass Care Navigators in Lexington, KY, is seeking a compassionate Social Worker for the PACE program. You will coordinate psychosocial services for participants and families, work with an interdisciplinary team, and help keep seniors living safely at home.

Schedule: Monday–Friday; provides community-based case management in the PACE Center, home, hospital, or ALF. Requires MSW CSWE-accredited and KY CSW/LCSW; two years in social work; strong networking and communication skills.

Qualifications

• Graduate of a master's program in Social Work accredited by CSWE and licensed in Kentucky as CSW or LCSW.
• Minimum of two years of social service experience in a case-management capacity.
• At least one year of experience working with the frail and elderly.
• Strong networking with community resources.
• Strong communication skills (face-to-face, telephone, computer).
• Detail-oriented and organized.
• Ability to interact with participants, families, and other agencies.
• Ability to manage multiple priorities and handle emotional workload.
• Ability to work unsupervised with discretion.

Position Summary:

Position Qualifications:

• Is a graduate of amaster’sprogram in Social Work accredited by the Council of Social WorkEducation;and is licensed by the state of Kentucky as a Certified Social Worker (CSW) or a Licensed Clinical Social Worker (LCSW).

• Ability to workunsupervisedexercising a high degree of discretion & independent judgment.
• Ability to physically access any home, hospital, nursing home, and ALF (i.e., navigate stairs, narrow stairs, narrow spaces, lack of air conditioning, etc.).
