Job summary

Qualifications

• Valid driver's license required.
• Current registration to practice as a Licensed Social Worker in New York State preferred.
• Master's Degree in Social work, or other Human Services-related field required.
• Minimum three years experience as an MSW working with patients in a mental health setting required.

Responsibilities

• Provides clinical supervision and direction to social work/behavioral health team members and assumes responsibility for overall program functions.
• Organizes program components for staff utilization including triage, case management, referrals, outreach, evaluations and liaison activities.
• Participates in quality assurance activities and ensures regulatory and contractual compliance.
• Screens referrals and performs psychosocial evaluation and assessment of mental health service needs and emergency social service needs of identified patients and their families.
• Develops and implements short-term service plans and provides crisis intervention services.
• Coordinates and follows up on linkages with other mental health and social services providers to ensure continuity of care.
