Job summary

VNS Health in New York, NY seeks a licensed mental health professional to provide direct clinical services, develop and update care plans, and lead complex care coordination. You will deliver supportive counseling, collaborate with community agencies, and maintain detailed EMR records to ensure quality patient outcomes.

Minimum two years of experience with Serious Mental Illness is required; LMSW/LCSW/LMHC or MFT licensure is needed or will be supported on hire.

Qualifications

• Master's degree in Social Work, Psychology, Mental Health Counseling, Family Therapy or related field required
• LMSW, LCSW, LMHC, or MFT licensure required or to be obtained upon hire
• Minimum of two years of mental health work experience with SMI and/or co‑morbid conditions, strong communication skills
• Strong oral/written/interpersonal communication skills; bilingual may be required.
• Basic computer skills and EMR familiarity.

Responsibilities

• Identifies client needs and develops clinical plans of care, updating as necessary
• Manages care for a caseload, leads care coordination for complex cases
• Provides supportive counseling and therapy, collaborates with agencies and refers as needed
• Develops resource inventories to meet client needs
• Connects clients to services and coordinates with providers and managed care plans
• Offers information and advocacy about entitlements and community services
• Maintains updated case records in the EMR and adheres to policies
• Participates in team meetings and discharge planning as appropriate
• Participates in 24/7 on‑call coverage as required
• Acts as liaison with community agencies
