Overview

Provides direct psychosocial services to mentally ill individuals in the community who are experiencing, or are at risk of, an acute psychological crisis or are in need of mental health treatment. Responsibilities include assessment, linkage, coordination with, referral to, and follow-up with appropriate ongoing service providers. Provides information and consultation to community agencies and other disciplines, including VNS Health services. Assists in the overall administrative and clinical functioning of the program. Works under general direction.

Our Mobile Crisis team members are on the front line of providing critical support to those we serve. We support our team with multiple layers of supervision, pairing team members in the field, weekly staff support groups, and comprehensive employee assistance programs. Key Responsibilities

• Provide clinical supervision to social work/behavioral health team members
• Organize program components including triage, case management, staff scheduling, and outreach
• Participate in quality assurance and ensure compliance
• Perform psychosocial evaluations and assessments
• Develop and implement short-term service plans

• Maintain case records and histories
• Encourage service-resistant clients to accept mental health services
