Job summary

Qualifications

• Licenses and Certifications: Licensed Social Worker in New York State preferred
• Education: Master’s Degree in Social Work or other Human Services-related field required
• Work Experience: Minimum three years experience as an MSW in a mental health setting required

Responsibilities

• Provides clinical supervision and direction to social work/behavioral health team members and assumes responsibility for overall program functions in the Program Manager's absence.
• Organizes program components for staff utilization and management, including triage and case management, staff scheduling, referrals, outreach, evaluations and liaison activities.
• Participates in quality assurance activities and ensures compliance with regulatory and contractual requirements.
• Screens referrals and performs psychosocial evaluation and assessment of mental health service needs and emergency social service needs of identified patients and their families.
• Develops and implements short-term service plans for patients with the team.
• Provides crisis intervention services and supportive counseling, case management and referrals for ongoing treatment.
• Maintains case histories and records per protocols.
• Encourages service-resistant clients to engage in services through interventions with clients and families.
• Participates in interdisciplinary team meetings and case conferences.
• Provides linkage to mental health and social services and follows up on connections for continuity of care.
