Job summary

Form Health is seeking a Senior Program Manager, Product to lead Product Operations in a fast-paced digital health environment. You will own the roadmap from idea to launch, champion AI-forward workflows, and ensure alignment across Product, Engineering, Clinical Operations and GTM teams.

This builder role requires autonomous prioritization, data-driven decisions, and ongoing refinement of the product operating model to accelerate value delivery and patient care.

Qualifications

• 5+ years in program, project management, or product operations in software/digital health.
• Proven experience building roadmaps or operating models from scratch.
• Fluency with AI-forward planning, coding and research tools and modern product tooling.
• Deep SDLC and Agile knowledge, adaptable to team needs.
• Strong data-driven decision making and cross-functional influence.

Responsibilities

• Own the Product Operations roadmap from vision to adoption.
• Lead end-to-end programs across clinical, platform, and go-to-market teams.
• Align Product, Engineering, Design, Clinical Operations, Marketing, and Customer Success.
• Pilot AI-forward tools and integrate them into planning and delivery pipelines.
• Design and improve the operating model, intake, triage, and rituals.
• Convert usage and clinical data into actionable roadmap recommendations.
• Provide executive health readouts with risks and recommendations.
• Identify and mitigate risks and blockers across teams.
• Serve as a trusted point of contact for stakeholders.
