Job summary

On Lok is seeking an Activity Leader to plan, develop, implement, and oversee activity services for participants in our PACE program. You will evaluate care plans, train aides, and coordinate monthly programs and community outings with the Activity Therapist and Volunteer Coordinator.

Ideal candidates have at least two years of related experience, experience with the elderly, and strong teamwork skills. Bilingual in Cantonese or Spanish is a plus; English fluency is required.

Qualifications

• Minimum of two years' experience in a social, recreational or educational program; includes at least one year in health care setting.
• 36 hours of training in activity leadership from an accredited college; or related therapy background.

Responsibilities

• Evaluates and implements participant care plans and trains aides.
• Plans varied monthly programs and uses group/individual activities to boost cognitive and functional abilities.
• Organizes monthly outings and special events with volunteers.
• Documents participant involvement and revises care plans as needed.
• Participates in team discussions and staff meetings; provides feedback.
